Submission of a New Cemetery Record to AAFA

This form allows you to enter data for submission to the AAFA Data Manager for inclusion in the AAFA Cemetery Databases. Please fill in the form as completely as possible, although we understand that in many cases, data may not be available.

Valid submissions are: Other Alford kin may be submitted, but may not be included in our cemetery data, however, it may be forwarded to one of our family branch managers for inclusion in the family branch genealogy.

You may move from field to field with your mouse or use the tab key to move from field to field. Shift-Tab will move to the previous field.

There are five sections of this form, submitter's (your) data, cemetery data, personal data of the deceased, family data of the deceased, and notes. The "submit" button appears at the bottom of the notes section.

Enter the name of the cemetery.
Do not use the word "cemetery" in the name.
In the table below, the fifty states and the District of Columbia are listed in alphabetical order. Beneath the name of each state is a pulldown menu that contains the entry, "Not Selected", the names of the counties in that State, and finally an entry for "Unknown" County. Find the cell that contains the State that the cemetery you entered above, and select the appropriate county name. You should only select one county name for this submission. Please note that the District of Columbia has only a single County, and that in addition to the Boroughs of Alaska, Named Census Areas (unincorporated regions) are marked with an asterisk.

In the text box at right, enter any cemetery notes that you think appropriate. Examples include alternate names for the cemetery, location, etc. If you need to refer to the AAFA cemeteries data already on this website, you may do so by clicking here. Enter the last name, first name(s), and suffix, if appropriate. Use standard capitalization and punctuation. For example, John H. Alford, Jr., would be entered as Alford for the Last Name, John H. as the First Name, and Jr. in the Suffix field. Mary Ann Alford Jones would be entered with Jones for the Last Name and Mary Ann Alford for the First Name.

Enter the dates of birth and death of the deceased. Select "Unk" if the day or month of birth is not known. If the year is unknown, enter ????. If you know that the birth or death year was before or after a certain time, you may so indicate with a < or with a >. For example, before 1850 would be entered as 1850<, or after 1900 would be entered as 1900>. For an approximate year use the Circa abbreviation CA. For example, a birth year of about 1845 should be entered 1845CA.

Enter the Social Security Death Index (SSDI) data here. The SSDI is available on the internet at several free sites, as well as on Ancesty.com. Enter the Social Security Account Number (SSAN), the USPS two letter abbreviation for the Issue State, the name of the county of the last residence, and the USPS abbrevition for the State of last residence.

Enter the family branch of the deceased by selecting the branch from the appropriate pulldown at the right. The family branch code is a tool that AAFA uses to identify ancestors. The family branch code consists of the first three letters of the ancestor's first name, the last three digits of his birth year, and the USPS abbreviation for his birth state. For example JOH807LA identifies John Seaborn Alford, born in 1807 in Louisiana. The family branch code is also the way we identify the genealogies on this website. The branch codes are divided into three groups, first names beginning with the letters A - I, those beginning with J, and those beginning with K - Z. Within each pulldown, the branches are ordered alphabetically by first name, then by year of birth.
