Below is a list of the death records from the AAFA master data base. Only those records for which there is proof of the death in Louisiana are included. For example the fact that a person is buried in a Louisiana cemetery is not proof the person died in Louisiana. A family group record submitted by a member reporting a Louisiana death is not proof without documentation. The intent was to provide data on persons born "Alford" but in some cases data may be included for persons whose married name was Alford.

The proof of death in these records came primarily from three sources:

1. The Social Security Administration, Social Security Death Index (SSDI)
2. Louisiana Statewide Death Index from ancestry.com
3. Published obituaries, many of which are published elsewhere in this issue.

Age information was available in most of the obituaries and death indexes. It was not available in the SSDI. If the age was not available in the source it was added by the computer by subtracting the birth year (not included below) from the death year.

Useful information can usually be found in the original application for social security. A copy of that application may be obtained by writing to the Social Security Administration.

To obtain the information it is suggested you reference "The Freedom of Information Act," 5 U.S.C. Section 552, request a copy of SS-5, Application for Social Security and include:

Name of the deceased
Social Security Number (SSN)
Date of birth
Date of death

The cost, at the time of this writing, is $27 so inlcude a check in that amount payable Social Security Administration.

Louisiana death certificates are identified in the list below in one of two ways:

(1) DC# followed by the volume and certificate number, or
(2) LDI (Louisiana Death Index) followed by the volume and certificate number.

To obtain a copy of a Louisiana certificate for a death more than 50 years ago, write to:
